The form is pretty basic except for a few things. The "Ch Reg" field is asking if your player is registered with the Clearing House. Please type in "yes" or "no". The "B/T" field is for "Bat" L/R or B and "Throw" L/R. The "Committed/JC is asking if the player is signed or committed to a college. Please answer "yes" or "no". Please list all players, even college players, as the college scouts do not know all players. One last thing to remember, this is a WYSIWYG form. In other words, we will NOT correct any spelling mistakes, so what you send us is exactly what you will get. After you submit this form we will email you a proof of your page, time permitting.
